Midwifery Week 2025: Getting Virtual Care Right: Three case studies exploring high-risk maternity care for the individual and group care patient

This presentation examines the implementation and outcomes of a virtual Patient-Centered Medical Home (PCMH) model designed specifically for high-risk maternity care. Through quantitative analysis and detailed case studies, we evaluate how digital health technologies enable comprehensive care coordination between virtual specialists and local in-person providers. The model demonstrates successful integration of behavioral health services, group care, medical nutrition therapy, and specialty consultation that maintains fidelity to relationship-based care principles and midwifery best practices.
